Titans Youth Sports Camp - WEEK 1

Cancellation Policy

Cancelations are only possible up to 72 hours before the first day of camp and will incur a $25 cancellation fee. ASSUMPTION OF RISK I understand that participation in athletic and recreational activities involves certain inherent risks, including but not limited to sprains, fractures, concussions, illness, dehydration, weather exposure, and other physical or emotional injuries. These risks may arise from the actions or negligence of myself, my child, other participants, staff, volunteers, or environmental conditions, including transportation to and from the Camp. I voluntarily assume all risks associated with my child’s participation in any and all Camp activities, including risks that may arise from negligence or carelessness on the part of Titans Youth Sports Camp, its organizers, employees, volunteers, sponsors, affiliates, facility providers, or agents. LOST OR STOLEN ITEMS I understand and agree that Titans Youth Sports Camp is not responsible for any personal belongings that are lost, stolen, or damaged during the course of the camp. Campers are encouraged not to bring valuables to camp. MEDICAL CONSENT In the event of an emergency, I authorize Titans Youth Sports Camp staff to seek emergency medical treatment for my child if I cannot be reached. I agree to be financially responsible for any costs incurred due to medical care or transportation. MEDIA By registering for Titans Youth Sports Camp, I grant permission for camp staff to photograph or record myself and/or my child during activities. These images may be used for promotional purposes, including social media, websites, and other materials, without compensation. If I do not consent, I will notify Titans Youth Sports Camp in writing before camp begins. INDEMNIFICATION I agree to indemnify and hold harmless Titans Youth Sports Camp and its representatives from any and all claims, actions, liabilities, damages, and expenses (including legal fees) resulting from or arising out of my child’s participation in the camp. By registering, I acknowledge that I have read, understood, and agree to the terms of this waiver and indemnification form.
